By clicking “I agree”, you confirm that you have read, understood and agree to the following, and you will waive certain legal rights, including the right to sue. Children’s Discovery Centre is not responsible for the children you are chaperoning, who must be in your care at all times.

IN CONSIDERATION OF Children’s Discovery Centre (“CDC”) allowing me and my child(ren) to enter and participate at the CDC, I acknowledge, represent, warrant and agree as follows on behalf of myself, my spouse (if applicable), child(ren), parents, heirs, assigns, personal representatives and estate, and to the extent that this Release applies to my child(ren) I confirm that I am satisfied that it is fair, just and equitable and agree that I will not revoke this consent during the minority of my child(ren) or thereafter:

 

I shall only enter the CDC as the chaperone of child participant(s), unless otherwise permitted by CDC staff. I confirm that I am eighteen years of age or older, and am the parent or legal guardian of all children entering the CDC with me or have been authorized to chaperone such child(ren) by their respective parent(s) or legal guardian(s), and agree to supervise such children at all times while attending at CDC.

 

I shall follow all posted CDC rules, as well as the instructions and directions of CDC staff and representatives, and require the child(ren) under my supervision to do the same.

 

I acknowledge that CDC does not represent or warrant that the picnic area at the CDC is free from allergens.

 

Release of Liability. I acknowledge that my, and my child(ren)’s, attendance and participation at the CDC entails known and unanticipated risks that could result in physical or emotional injury, paralysis, death, or damage to myself or my child(ren), to property, or to third parties (the “Risks”), and I hereby release and agree to defend, indemnify and hold harmless CDC. its employees, successors and assigns from any and all liability, claims, demands, injuries, lawsuits, causes of action, death, losses, costs and expenses in connection such attendance and participation which I or my respective heirs, executors, administrators or next-of-kin may now or hereafter have against them or any part of them. I understand that the Risks cannot be eliminated without jeopardizing the essential qualities of my or my child(ren)’s participation at the CDC and I expressly agree to accept and assume all Risks on behalf of myself and my child(ren). My and my child(ren)’s attendance and participation at the CDC is purely voluntary and I elect for us to attend and participate in spite of the Risks.

 

Publicity Release. I acknowledge that CDC may take photographs of individuals attending at the CDC, and I consent to CDC’s use of my or my child(ren)’s picture and likeness in any advertising and publicity CDC and its successors, assigns and licensees may conduct relating to my or my child(ren)’s attendance at and participation in the CDC in any media or format, whether now known or hereafter developed, including but not limited to the World Wide Web, at any time or times in perpetuity, without further compensation or notice, and I hereby release CDC and its assigns from any liability with respect thereto.

 

CDC reserves the right to deny entry or eject me or my child(ren) in the event that I or my child(ren) are not in compliance with the terms and conditions stated herein, or fail to comply with any posted rules, or the instructions and directions of CDC staff and representatives.
